New and Returning Patients: You can schedule a new and returning patient appointment through the phone or online portal. Check with the receptionist to see whether your appointment is online, or in a physical office location before the day of your appointment. If it is an office visit, make sure to verify in what location. For new patient appointments, you are advised to arrive 15 minutes before your appointment, unless prior arrangements have been made. Returning patients are advised to be on time for their appointments. Important Information
Cancellations or reschedules must be done 48 hours before the day of the appointment. ‘No shows’ or ‘missed’ appointments for Follow-up appointments are charged a $100 fee, which must be paid in full before the patient will be seen again for any visit. There are no refunds for Initial Evaluations. (Please, remember that when you miss an appointment without a 48-hour notice, you are wasting a valuable resource that can help save someone else’s life that desperately needs it).
Refills: Refills are done during an appointment visits only, and we will provide enough medication to get the patient to the next appointment. It is the patient’s responsibility to make sure that they schedule and show up for their follow up appointments, so that they do not run out of medication. Patients can be given refills in special events without an appointment, and it is up to the provider to decide what those special events are. Not showing up or missing your appointment is not considered a special event.
